Our organization aims to provide safe, affordable, and supportive housing options for veterans and seniors. Our programs include housing assistance program, health and wellness resource services and community support initiatives.

3. Eligibility

To qualify for our housing programs, applicants must meet the following criteria:

- Be a veteran or senior (age 45 and above).

- Meet defined income eligibility requirements.

- Complete the application process (housing referral intake form), including submission of necessary documentation.

4. Application Process (Referral Intake Form)

To apply for housing, interested individuals must:

- Complete the application (Referral) form available on our website.

- Provide any required documentation as specified in the application or requested via email..

- Submit the application to info@forwardlivingsolutions.

Approval of applications is at the sole discretion of Forward Living Solutions.
We reserve the right to deny applications based on eligibility criteria.

5. Donations

Donations to support our programs are welcomed and appreciated. All donations are tax-deductible to the extent allowed by law. By making a donation, you agree that:

- Donations are non-refundable.

- You will receive a receipt for tax purposes.
